• CBO Capacity Assessment Tool—helps HCOs and MCOs rapidly engage, and more effectively partner with, local CBOs. The tool provides a complete capacity and needs report for each CBO and an aggregate report for a full CBO network.
• CBO Engagement Training Series–provides CBOs with the information they need to develop their value proposition, identify outcomes that are meaningful within the healthcare system, inform their IT planning efforts to track the right metrics and return on investment, and build service rates that cover the cost of care.

Webinars:

• Pathways HUB: A Population Health Model that Activates the Community Response to Social Determinants—This webinar shared The Pathways Community HUB model, designed to help coordinate outreach by specialized community health workers who are incentivized to engage high-need populations. Speakers shared ways community hubs can adapt to address broader community needs for an array of populations and be funded by performance-based managed care payments. The webinar was presented by HMA and the model’s co-developer.
• Advancing and Codifying Health Equity and Wellness for Medicaid Populations—During this webinar, experts discussed a recent Academy Health issue brief outlining lessons learned from CBO partnerships and examine the role CBOs play in addressing  social determinants of health to improve health disparities and outcomes.  The webinar was presented by HMA and the brief’s co-authors.
